Summer Shootout Clinic - Registration

Three ways to register:

1. Register & Pay Online - Use the form below to register. After submitting the form, use the Paypal buttons to pay online. Download the Medical Release Waiver (link below) and email it to becky@summershootout.org.

2. Pick-up a Brochure at Fort Myers High School - Mail in your registration form and check payable to Summer Shootout to: Summer Shootout, 3004 Cortez Blvd, Fort Myers, FL 33901

3. Print & Mail - Print the registration form (link below) and mail it, along with check payable to Summer Shootout, to: Summer Shootout, 3004 Cortez Blvd, Fort Myers, FL 33901.

REFUND POLICY: Summer Shootout clinic registration may be cancelled up to two weeks before the first day of camp. We are not able to give refunds for cancellations with less than two weeks' notice. A $15 administrative handling fee per child per camp will be assessed for all cancellations and transfers. To cancel or make a transfer, please email becky@summershootout.org. If your child is unable to attend camp due to medical reasons, you may receive a full refund (minus a $15 handling fee) at any time prior to the start of camp by providing a doctor's note. We are unable to refund for missed days of camp.
